Steps to Streamline Your First Visit

Please set aside some time to complete the required paperwork. You will receive an electronic link to the initial paperwork upon scheduling your appointment. Your completed intake form will be electronically submitted to your practitioner and will allow her to review your health history prior to your visit.

If you prefer a paper version, please contact the clinic, we will happily mail or email you the paperwork. Or please show up to your appointment 15 minutes early to complete.

1. Complete the appropriate New Patient Intake Form

This will allow the doctor to become oriented to your history and your health needs, and it will maximize what can be accomplished in our visit.

2. (Optional) Bring or Request Copies of Medical Records

If you have medical records (labs, x-rays, etc.) from current and/or previous providers that would help us with your care, please request and complete the Medical Records Request form. We will use this form to request these records form the appropriate source.

We are considered “out-of-network” providers for all insurance companies. Many insurance companies will reimburse their customers for the full or partial cost of a visit with an out-of-network provider. Our clinic does not bill insurance. We will provide you with a superbill, which you can submit to your insurance company to request reimbursement.
